Your occupational therapist conducts in-home and community-based assessments to understand how your health condition, environment, or circumstance is affecting your ability to manage daily activities. Assessments inform recommendations, funding applications, and care planning, and give your team the clinical foundation to act.
We identify and address the physical barriers in your home that create risk or reduce independence. This includes recommending and coordinating modifications, grab bars, ramps, bathroom adaptations, and spatial reorganization, so your environment works for you, not against you.
Our OTs assess, recommend, and support the sourcing of adaptive equipment and assistive technology suited to your functional needs. From mobility aids to communication devices to daily living tools, we match the right equipment to the right person and context.
We work directly with individuals to rebuild or adapt the skills needed for daily living, personal care, meal preparation, household management, community access, and return to work or school. This is hands-on, goal-directed work carried out in the settings where those skills actually need to function.
